CONSENT TO PERSONAL DATA PROCESSING

I agree that the administrator JAMES DEAN CO., s.r.o., with its registered office at Charvátova 1988/3, 110 00 Prague 1 - Staré Město, Czech Republic, Identification No. (IČ): 24229814 (or its employees) collects, keeps and processes the personal data contained in this form for the purpose set forth below. I give this consent in respect of all data contained in this form for a period of 5 (five) years following the date when the consent was given. I am aware of my rights pursuant to Sections 12 and 21 of Act No. 101/2000, Coll., on Personal Data Protection. I have been acquainted with all filled in parts of this form and all data are accurate and true and have been provided voluntarily.

Purpose of Personal Data Processing

In compliance with Section 5 of Act No. 101/2000, Coll., on Personal Data Processing, all data stated in this form are to be collected and processed solely for the purposes of sending newsletters to the data subject electronically pursuant to Act No. Sb. 480/2004, Coll., on Certain Services of Information Company; such newsletters shall be sent until the data subject directly and effectively sends information to the administrator that the data subject no longer wishes that the administrator sends newsletters to him/her, but for no more than 5 (five) years. The administrator may use summarized data from this form for statistical purposes for its own needs.

Administrator’s Declaration

The administrator declares that it shall collect personal data in the extent necessary for the fulfilment of the above-mentioned purposes and shall only process the same in compliance with the purposes for which the data were collected. The obligation on the part of the administrator’s employees or any other individuals who process personal data under an agreement with the administrator or on the part of any other persons to keep personal data confidential shall survive the termination of the employment or the relevant works.
